- Retrocaecal is the most common position
- classic Murphy triad is almost diagnostic - Pain - vomiting - fever occur in same sequence
- appendicular perforation is more common in elderly and children this is because atherosclerosis of small vessels in elderly and narrow lumen in children . Hence early diagnosis and management is required in children as delay in diagnosis lead to peritonitis
